PROCARE - Primary prevention in longterm care
Considering the latest developments of demographic change as well as the increasing demand for geriatric care takers this project aims to provide employees (care takers and employees of domestic economy) and residents of stationary residential care home for the elderly with a prevention program based on physical activity. The program is performed subsequent to an individual determination of requirements using the BASE-concept (Wollesen, Wenzel, Lex & Mattes, 2016). The following goals are part of the PROCARE-program: facilitating the individual health condition and mobility, developing resources persons with multiple morbidities, implementing the setting approach and focussing on the care takers by overall participation. Six residential nursing homes for elderly in different districts of Chemnitz and rural surroundings are addressed with these goals. The overall lead of the cooperative project rests with the University of Hamburg, with six further German Universities being involved (Frankfurt, Erlangen-Nürnberg, Karlsruhe, Stuttgart, Paderborn and Bremen). Legal basis for the project is the care-taking prevention act (§5 of SGB XI).
